Please send your application in electronic format via (please copy and paste the WeTransfer link to the email), OneDrive or Google Drive to

Please check to ensure that your submission includes the following:

  • Completed Application Form (AD1)
  • Professional Report (AD2) (with the required signatures)
  • Overview of Chartership Criteria (AD3)
  • CPD records (AD4)
  • Supporting Documents with appropriate cover notes (SD 1-6)   

A check list is provided at the end of the application form.

Sponsors forms

The two Sponsors’ Statement Forms (AD5) should be sent, by your Sponsors, directly to the Society. 

It is your responsibility to provide your Sponsors with the form and ensure that they are sent to the Society.


After we close the submissions we will contact you to arrange the payment.

What happens next?

On receipt of your application an acknowledgement will be sent. If your application is incomplete for any reason your interview may be postponed to a later date.

With the current difficulties of holding face-to-face meetings for Chartership, we are currently conducting Chartership interviews via video conferencing (Zoom or Microsoft Teams). Fellows wishing to apply for Chartership should continue to do so as normal.
